The Importance of Advance Care Planning


People who have never talked with their families about their medical choices at end of life are at a disadvantage in a medical crisis. The lack of Advance Care Planning (ACP) written down in a document called an Advance Directive could result in stress for family members at an already-difficult time as well as other adverse effects.

 

This presentation will inform you about the importance of ACP and explain the parts of the SC Healthcare Power of Attorney form, which allows you to choose someone to speak for you about your healthcare decisions and also determine what healthcare decisions you would prefer.
